It is with great enthusiasm that we welcome you to the sixth edition of the Video Journal of the ESGE Academy.
This initiative was created with a clear and ambitious purpose: to serve as a comprehensive and enduring collection of high-quality video content dedicated to all aspects of endoscopic practice. Whether you are learning a technique for the first time or seeking to refine an established method, this journal offers clear, structured, and accessible educational resources to support your growth as an endoscopist.
Unlike much of the video material shared on social media—where we often see only the most advanced techniques performed by highly experienced practitioners or novel devices used in complex, edge-case scenarios—the Video Journal of the ESGE Academy offers something different. We focus on what lies beneath the surface of the iceberg: the essential techniques, fundamental procedures, and core knowledge that form the foundation of high-quality endoscopy. These are the building blocks that every practitioner, regardless of their experience level, must master.
Any endoscopic procedure can be featured here. Whether it is a groundbreaking innovation or a time-tested method dating back to the earliest days of endoscopy, we welcome it—provided it is presented with the clarity and structure that enable any viewer, regardless of prior exposure, to understand and apply it in their own practice. Each contribution should break the technique down into its constituent steps, illustrate the critical elements, and communicate the key concepts in a way that is practical, accessible, and immediately useful.
The Video Journal of the ESGE Academy will be published quarterly, with themes and topics announced three months in advance. If you are passionate about education and have a technique or concept to share, we warmly encourage you to submit your video for review. Submissions that meet our editorial standards and are selected for publication will be proudly showcased here on our platform.

Sixth Edition: Materials and equipment
This issue is about the things we pick up — not the technique, the tools. Snares, needles, wires, stents, and the current that runs through some of them. Three groups, describing three very different aspects of endoscopy, arrive at the same conclusion: devices are not interchangeable, and the properties of the device in your hand (or down your endoscope) may decide the outcome as much as the technique you apply.The lower GI group takes us through the polypectomy kit, where the wire decides the plane and a clip too narrow for the defect is no clip at all. The electrosurgery segment explains how cutting and coagulation are the same current, delivered differently, and how this has a significant impact on the tissue effect. Therapeutic EUS shows device selection at its most unforgiving, where the working channel sets everything downstream and every device chooses the next one.Three videos, one rule. The device you are using can dramatically determine the effect of your therapeutic endoscopy manouevre. Know the material in your hand to maximise your therapeutic outcomes.

Electrosurgery in the Endoscopy Unit
Principles, devices and settings
This video shows how radiofrequency current can be used for cutting or coagulation and explains how to select the appropriate settings for each device. The lecture covers monopolar and bipolar circuits, current density and tissue temperature thresholds, spark formation and impedance, waveform and impedance-guided outputs. It then goes through EMR, ESD, saline immersion, haemostatic forceps, argon plasma coagulation and biliary sphincterotomy.

What to select, and how to use it: the polypectomy toolkit
Special Edition: This 6th edition of the ESGE Academy Video Journal is produced in collaboration with the ESGE Young Endoscopists (EYE) committee.
This presentation focuses on the materials and equipment used for polypectomy and endoscopic mucosal resection (EMR) in the lower gastrointestinal tract. It reviews the properties of key devices — cold snares, injection needles and submucosal fluids, types of snares for hot snare polypectomy and EMR, electrosurgical units and different settings, and haemostatic devices — and explains how these properties determine the resection plane, completeness of excision, and safety. Practical guidance is given on matching device selection to lesion type and size, with reference to the ESGE curriculum for training in EMR in the colon and the 2024 ESGE colorectal polypectomy and EMR guideline. Case-based video examples are used to demonstrate the correct use of key materials, including cold snare capture, dynamic submucosal injection, hot snare transection, and snare-tip soft coagulation.

Characteristics of different endoscopic equipment in EUS
The therapeutic EUS toolbox
Therapeutic EUS depends on understanding what each device is made of and why that construction fits a given task. This session walks through the toolbox in the order of the procedure itself: the echoendoscope and the working channel, access needles and the guidewires, devices for creating the transmural tract, and the full range of drainage stents — double-pigtail plastic stents, lumen-apposing metal stents and dedicated partially covered SEMS for hepaticogastrostomy — before closing with the material used for vascular embolisation. For each family, the focus is on the three variables that actually change behaviour at the table: calibre, stiffness and delivery system. The aim is a practical framework for choosing equipment, so that the reasoning survives the next change in the catalogue.
