EUS in postsurgical fluid collections

COURSE
Series: ESGE Days 2025
Season: Video sessions EUS: EUS drainage in complex cases
Episode 2 of 3: EUS in postsurgical fluid collections

SPEAKERS
Dr. Sundeep Lakhtakia

DESCRIPTION
Dr. Sundeep Lakhtakia speaks about the diagnostic and management of postoperative fluid collections (POFC), focusing on the EUS-guided draiange. Most often, POFC occur after pancreatic surgery (especially after distal pancreatectomy), but also after colorectal, hepatico-biliary, bariatric and pelvic surgery. The EUS-guided drainage is a safe treatment option, it should only be performed if the patient is symptomatic. CT- imaging is paramount before drainage, because it evaluates the location, the extension as well as the characteristics (fluid vs. necrotic. vs. hematoma) of the fluid collection. Metal as well as plastic stents can be used.

LEARNING OBJECTIVES

Extreme luminal anastomosis: colon and other targets (00:15:44)

Description
Prof. Marc Barthet illustrates various EUS-guided luminal anastomosis techniques using numerous clinical case videos, covering both malignant and benign pathologies (i.e. gastroparesis, complications after bariatric surgery such as twist post-sleeve gastrectomy). He shows videos of EUS-guided sigmoido-ileo anastomosis for malignant colonic stenosis, EUS-guided colo-jejunal anastomosis for peritoneal carcinomatosis, EUS- guided jejuno-jejunostomy for candy cane syndrome after total gastrectomy as well as EUS-guided gastro-enterostomy for complications after bariatric surgery. Practical insights and technical recommendations are provided throughout. He underlines that there is still a lack of data (especially in the post-procedural management) on this topic.
