Standardizing EUS-guided Gastroenterostomy: a Delphi consensus on technical steps and adverse events management (00:09:21)
Description
Dr Vanella presents a study for whicht the Delphi methodology was used in order to establish expert consensus on key aspects of EUS-guided gastroenterostomy (EUS-GE), a procedure increasingly used for managing gastric outlet obstruction. A panel of 25 international experts reviewed literature and participated in three rounds of predefined statements related to technique, safety, and procedural preferences. Strong agreement was reached on the use of fluoroscopy, electrocautery-enhanced lumen-apposing metal stents (LAMS), free-hand LAMS release, patient positioning, sedation, and saline solution for jejunal distention. Moderate agreement emerged on aspects like catheter-assisted EUS-GE and procedural space requirements, while statements regarding contrast use and technique preferences were ultimately removed due to a lack of consensus. The findings highlight key technical principles for standardizing EUS-GE while identifying areas that require further research.

Video live endoscopy: Tips and Tricks for biliary cannulation (01:03:36)
Description
Biliary cannulation techniques are crucial for successful ERCP procedures. The session covers essential anatomy of biliary and pancreatic ducts, emphasizing practice for skill enhancement. Key techniques like guidewire manipulation and sphincterotomy are discussed, along with advanced methods such as the double guidewire technique. Experts share insights on managing difficult scenarios, including a case study on bile duct stones and the increasing use of EUS biliary access.

EUS in postsurgical fluid collections (00:19:53)
Description
Dr. Sundeep Lakhtakia speaks about the diagnostic and management of postoperative fluid collections (POFC), focusing on the EUS-guided draiange. Most often, POFC occur after pancreatic surgery (especially after distal pancreatectomy), but also after colorectal, hepatico-biliary, bariatric and pelvic surgery. The EUS-guided drainage is a safe treatment option, it should only be performed if the patient is symptomatic. CT- imaging is paramount before drainage, because it evaluates the location, the extension as well as the characteristics (fluid vs. necrotic. vs. hematoma) of the fluid collection. Metal as well as plastic stents can be used.
